A Model of Care

Strive has developed the CREATE Model of Care to provide a clear and purposeful framework for interactions between children, young people, their families, and care teams.

The CREATE model is grounded in Contemporary Trauma Theory, Social Model of Disability, and Self-Determination Theory, and has been influenced by the learnings from the Adverse Childhood Experiences (ACE) Study, and the Australian Child Maltreatment Study (ACMS).

The model uses the secure base of a stable home environment and a well-structured care plan to reignite the developmental progress disrupted by past trauma and/or supporting disability-related needs in a safe and capacity-relevant way. Our goal is to empower those we support to develop crucial capacities and seize opportunities as they embark on their growth journey.

Transforming Trauma Through Relationship-Centric Support

CREATE is a domain-focussed capacity-building model of care designed to leverage off the therapeutic benefits of a well-structured case plan and the secure-base (home that is safe, constant and predictable) of a stable placement to position the relationships that the child will form with Strive carers to re-ignite the development that was disrupted when the trauma began.

The CREATE Model of Care is designed to take place within a residential setting and position children and young people to develop the below capacities and opportunities in their healing journey.

Connection

We prioritise the development of safe and meaningful relationships as the foundation of our therapeutic work.

Regulation

Our core care team provides therapeutic support to help assess and develop regulation (self-soothing) abilities.

Explore

We create and facilitate opportunities for young persons/participants to discover and develop their gifts, talents, and aspirations whilst establishing a local, holistic support system.

Acquire

Through structured routines and repetitive exposure, young persons/participants gain essential life, vocational, and educational skills, building competence and confidence for future independence.

Tolerance

We foster the capacity to tolerate stressors and challenges as resilience is key to sustainable outcomes. This is achieved through exposure to diverse cultures, ideas, and ways of thinking, crucial for developing strong relationships, along with healthy risk-taking.

E

Effect

This final stage integrates all acquired skills and strengths to inform a comprehensive transition plan, preparing the young persons/participants for their next home.
