A Model of Care
Strive’s CREATE Model of Care has been developed to provide guidance, direction and purposeful interaction between the child, young person and their care team. Its design has been directly guided by Contemporary Trauma Theory.
CREATE is a domain-focussed capacity-building model of care designed to leverage off the therapeutic benefits of a well-structured case plan and the secure-base (home that is safe, constant and predictable) of a stable placement to position the relationships that the child will form with Strive carers to re-ignite the development that was disrupted when the trauma began.
The CREATE Model of Care is designed to take place within a residential setting and position children and young people to develop the below capacities and opportunities in their healing journey:

Our model places the development of safe and meaningful relationships with the child and young person at the centre.

egulation
The opportunities to assess and develop regulation (self-soothing) is a core focus of intervention through therapeutic support from the core care team.

Creating, planning and accessing opportunities that without a set focus can be missed, leaving young people ill prepared to explore their gifts, talents and aspirations.

cquire
Through the repetitive (structured), patterned and rhythmic (routine) exposure to life skills, vocation and education, children and young people will acquire the skills needed to position them with the competence and confidence required to thrive when they living in another care environment or independently.

Tolerance is the capacity to accept differences in others and is a key component in developing and sustaining relationships. Targeted and planned exposure to different cultures, foods, ideas and ways of thinking will be an inherent component of the support all children and young people can expect.

The integration and coordination of all the skills and strengths acquired/ identified in previous stages to inform an informed transition plan that sees the child or young person transition from their home with Strive to their next home.
